1.  Open the Credit Card Form [CLICK HERE] and complete it - You can TYPE right in the  WEB PAGE that opens.  The PRINT the Form, then SIGN and DATE it.

2.  Fax it back with your RENTAL AGREEMENT to 212.419.2340.  Please note, you will be Faxing to a Secured Voice Mail box that only we have access to.

3.  We charge your card per your instructions and send you a confirmation of the payment application.
